Invitation to Community Health Needs Assessment Town Hall

The PATH Foundation, Fauquier Health and the Rappahannock-Rapidan Health District are currently conducting research for the 2017 Community Health Needs Assessment (CHNA).  This research will be used to develop an action plan to better health for Fauquier, Rappahannock and Culpeper Counties by identifying our community’s significant health needs and priorities.
On Thursday, April 6, a Community Health Town Hall will be held in the Fauquier Hospital Sycamore Room as a key step in this wide-ranging process to identify significant health needs.
What’s on the horizon for the 2017 Health Needs Assessment? We need to hear community voices to help understand community challenges.
Please join us at the CHNA Town Hall to learn more about the health needs assessment and to share your own thoughts on the health of our community.  Details are below:
